How do I know which laser lens my PS4 needs to repair the drive?

To identify the correct laser lens for your PS4, you need to know the console model (CUH number on the rear label). PS4 CUH-1000/1100 use KES-860A lens, CUH-1200 use KES-490A, and PS4 Slim (CUH-2000) and Pro (CUH-7000) use KES-496A. If only the lens is damaged you can replace it individually; if the complete mechanism fails, it is better to replace the entire Blu-ray drive.
